Though diagnostic imaging by MRI or CT Scan has become the norm in most specialities, it has not become so very popular in strabismology, inspite of the fact that high resolution imaging as is available now, provide lot of information in causation od strabismus and the sequele of long standing strabismus.MRI in esohypotropia of high myopia has shown the pathogenesis of the sstrabismus which has revolutionised the surgical technique as shown by Yokoyama in 2000. Demer has shown that heterotopy of pulleys play an imprtant role in the causation of incomitant strabismus.Management of complex strabismus as in restrictive strabismus like Duanes retraction syndrome , thyroid ophthalmopathy ,Moebius syndrome and some of the paralytic strabismus like long standing third nerve palsy is very challenging and imaging provides useful tips to plan surgery well ahead. Here we discuss some of the complex strabismus cases where diagnostic imaging provides foresight into its management.
