Abstract
Cataract surgery in presence of small pupils offers a significant challenge to any surgeon, but can especially be daunting to a less experienced surgeon. It is a humbling experience, admitting to being uncomfortable and having to refer elsewhere. In this course, we discuss common causes and evaluation of such cases followed by the different surgical technical options in managing small pupils. We cover both MSICS and phacoemusification surgeries. Among surgical techniques, we include management of phacoemulsification without additional pupil dilatation techniques, as well as the various pupil dilating techniques of stretch pupilloplasty, iris hooks, Malyugin and B Hex rings. We approach it in such a way as to guide a surgeon uninitiated into these techniques into performing them. Experienced cataract surgeons use their surgical videos to instruct the audience into learning how to handle the challenge of intraoperative floppy iris and small pupil cataract surgery
