Taming the Iris in Cataract Surgery

Abstract

Iatrogenic iris defects are most frequently caused by cataract surgery. The fragility of the iris , its proximity to the instruments and currents of cataract surgery and at times the flaccidity of the iris, make iris trauma inevitable, even though infrequent. Every cataract surgeon should be well versed with the techniques to prevent or minimize the damage to the iris when it decides to misbehave. The importance of a thorough preoperative evaluation, role of associated comorbidities, role of pharmacotherapy, role of OVDs and surgical strategies to minimize the negative spiral of escalating iris damage will be explained in detail in this instruction course. When damage occurs, the surgeon must decide if , when and how to repair the iris. With adequate knowledge of the correct techniques one can greatly reduce the incidence and severity of iatrogenic iris damage.
